The 3-Month Health Transformation is a comprehensive wellness program designed to help you make meaningful and sustainable changes to your health in just three months. This program offers personalized coaching, expert guidance, and tailored support to help you reach your goals—whether that’s weight loss, increased energy, or better stress management. What’s included: 12 weekly coaching sessions: You’ll have one 60-minute coaching session each week, during which we’ll work together to address your health challenges and develop a plan that fits your lifestyle. Each session is focused on your specific needs, from nutrition and meal planning to exercise and mental well-being. Custom meal planning: We’ll create meal plans that are specifically designed for your dietary preferences and goals. These meal plans will evolve over time, based on your progress and feedback. Stress management techniques: Learn effective ways to manage stress and improve mental clarity through personalized strategies that fit your lifestyle. This includes techniques like mindfulness, time management, and relaxation exercises. Grocery shopping guidance: You’ll receive expert advice on how to navigate the grocery store, choose healthier options, and build better shopping habits to support your wellness journey. Ongoing accountability: Between sessions, we’ll stay in touch via email or text, offering encouragement, guidance, and advice to keep you on track. At the end of three months, you’ll see significant improvements in your health and well-being, with sustainable strategies in place to maintain your progress. This program is perfect for those ready to commit to real change and long-lasting results.
Cancellation Policy
1. Payment Policy All services must be paid in full prior to the start, unless a payment plan is arranged. We accept major credit cards, PayPal, and bank transfers. Payment plans are available for long-term programs. 2. Cancellation & Rescheduling A minimum of 24 hours' notice is required for cancellations or rescheduling. Cancellations within 24 hours will be charged the full session fee. Sessions can be rescheduled once with 24 hours' notice. No-shows will result in the session being forfeited. 3. Refund Policy No refunds are given for completed services. For long-term programs, partial refunds for unused sessions are available if canceled within 14 days of the start. No refunds after this period. Corporate wellness programs are non-refundable once the first session begins. 4. Scope of Practice Polat Health provides educational services and does not offer medical advice, diagnosis, or treatment. Clients should consult their physician before making significant lifestyle or dietary changes. Polat Health is not liable for any health complications arising from changes made without professional medical advice. 5. Liability Waiver Clients assume full responsibility for their health and well-being. Polat Health is not liable for any adverse outcomes related to its services. Results vary, and success depends on client commitment. 6. Confidentiality All client information is confidential and will not be shared without consent. Personal data and session details are securely stored. 7. Client Responsibilities Clients are responsible for participating in sessions, completing tasks, and making their best effort to implement the coaching strategies. Missed or repeated absences may lead to termination of the program without a refund. 8. Program Termination Either party may terminate the program with written notice. Refunds follow the terms outlined above. Polat Health reserves the right to end a program if clients repeatedly miss sessions or fail to participate. 9. Corporate Wellness Corporate wellness programs are customized per contract. The company is responsible for promoting employee participation. Payments are non-refundable once the program begins. 10. Free Health History Session This session is a one-time offer for new clients. It may be rescheduled once with 24 hours' notice. Missed sessions without notice will result in forfeiture of the offer.
